The SMBP Study

Purpose of this Study

You will get your own blood pressure monitor to use at home. We will teach you how to use it. You will check your blood pressure at home and share the numbers with your doctor's team for 12 months. The research team will also collect information in other ways, like: - Visits in person - Phone calls - Your medical records - Medicare data

Who Can Participate?

Eligibility

Adults age 65 or older who live on their own (not in a nursing home or hospice). Have high blood pressure: average top number (systolic) is 145 or higher over the past 6 months. Have at least one other heart-related health problem.

What is Involved?

Description

We want to compare two ways to help older adults check their blood pressure at home. One way includes help from a pharmacist, and the other does not. We want to see which way works better for lowering blood pressure in people who have more than one health problem.
